Healing Bridges is a recovery-oriented nonprofit that guides and connects residents of Washington County and neighboring communities to comprehensive behavioral health and developmental health services.
We are looking for a full-time Intellectual Disabilities & Autism Supports Coordinator to join our team.
This position reports to our site location in Washington, Pennsylvania and regularly attends client appointments and meetings in the community around Southwestern PA (mainly Washington, Allegheny, Greene / Fayette County).

As a Support Coordinator, you will play a vital role in assisting individuals with developmental disabilities and behavioral health needs.
This is an excellent opportunity for someone with a background in social work or program management who is passionate about making a positive impact on the lives of those with disabilities and delays.
The ideal candidate is someone who is ready to show up and can be an advocate for people diagnosed with intellectual and developmental disabilities and their families.

Conduct in-depth initial assessments of clients to determine the nature and extent of each person and family's needs and provide a provisional diagnosis with an individual service plan (ISP).
Coordinate and monitor the delivery of vocational, medical, social and educational services through natural supports, generic community resources, specialized mental health treatment and rehabilitation assistance to ensure client needs are met.
Manage a caseload of up to 50 clients within the geographical region of Washington County, Pennsylvania and other neighboring counties while maintaining the agency's productivity standards.
Provide support, guidance and consultation to clients when accessing community resources and service options.
Make referrals to available resources and eligible waiver programs when necessary.
Collaborate with other professionals such as therapists and healthcare providers to ensure comprehensive client care regarding services, liability, payment issues and license requirements.
Maintain accurate and up-to-date client health records using the PA State Home and Community Services Information System (HCSIS) and Credible, the agency's electronic health record.
Skills and
Qualifications:
Act 33/34 & FBI Clearances required.
Minimum:
Bachelor's degree in related field required.
